MSL: Thank you for taking the time to meet with me today, Dr. Johnson. I wanted to discuss our company's JAK inhibitor and its data in atopic dermatitis.

Dr. Johnson: Thanks for coming. I've been following JAK inhibitors in dermatology closely. The efficacy data looks impressive, but I still have some safety concerns given what we've seen in the RA space.

MSL: That's an excellent point. Our phase 3 program included extensive safety monitoring. The MACE rates were not significantly different from placebo in our integrated safety analysis across all trials. The most common adverse events were nasopharyngitis and upper respiratory infections, which were generally mild to moderate.

Dr. Johnson: What about thromboembolism risk? That's been a concern with other JAK inhibitors.

MSL: Our data showed a rate of 0.3 events per 100 patient-years, which is within the background rate for the atopic dermatitis population. We did exclude patients with high thromboembolic risk from the pivotal trials, so that's an important consideration for real-world use.

Dr. Johnson: And what about lab abnormalities? Do you see the lipid elevations and cytopenias that have been reported with other JAKs?

MSL: We do see modest, dose-dependent increases in LDL and HDL, typically peaking at week 8 and then stabilizing. For hematologic parameters, we observe a small decrease in platelets and neutrophils, but grade 3-4 laboratory abnormalities were rare, under 1% in the clinical program.

Dr. Johnson: That's helpful context. In my practice, I've been seeing more severe atopic dermatitis patients who have failed multiple therapies including dupilumab. Would there be any data on efficacy in this population?

MSL: We have a specific subgroup analysis from our phase 3b study that included patients who had inadequate response to biologics. In this population, we saw EASI-75 rates of 42% at week 16 compared to 8% for placebo. Not as high as in biologic-naïve patients, but still clinically meaningful.

Dr. Johnson: Do you have any data on combination with topical corticosteroids? That's how I typically manage most patients.

MSL: Absolutely. Our phase 3 program included a study with concomitant topical corticosteroids. We saw enhanced efficacy with combination therapy – EASI-75 rates increased from 63% with monotherapy to 78% with combination at week 16, with no significant increase in adverse events.

Dr. Johnson: That's good to know. What about rapidness of response? My patients are often looking for quick relief of symptoms.

MSL: We observed significant improvements in itch as early as day 2 in our phase 3 studies. For skin clearance, meaningful improvements were seen by week 2, with continued improvement through week 16.

Dr. Johnson: Excellent. I'm currently involved in a registry study for atopic dermatitis. I'd be interested to know if your company has any ongoing registry or real-world evidence programs planned.

MSL: We're launching a global registry later this quarter that will follow patients for 5 years. The focus will be on long-term safety and effectiveness across different patient populations. We're actively looking for investigator sites if you'd be interested in participating.

Dr. Johnson: I would definitely be interested. Could you send me more information about the registry requirements and protocol?

MSL: I'll send that over this week. Before I leave, do you have any other questions about our JAK inhibitor or the data package?

Dr. Johnson: I think we've covered most of my questions. I appreciate the scientific discussion and look forward to seeing the registry information.
